If you haven't got a Google account you need to set one up with a Gmail email address.
Go to : http://www.youtube.com
Click on Login top right, then Create Account.
Click on I would like a new Gmail address
Complete all fields, once complete you will be sent an email to your current email address to acknowledge this new Google account.
Login to Youtube : http://www.youtube.com/
Play the YouTube video you would like to save, the click Add to, then create new playlist, type a name you would like to call the folder, then press create
This will only create the Playlist folder you will need to go back to the video and select Add to, then select the newly created folder again to save.
Saving playlists with an iPhone is the same as above play the video, click on + sign (Add to) , then add to Playlist, select a playlist folder or create a new one.
